Only market makers can make transactions outside of trading hours for OTC stocks. What happened this morning is that one market maker traded shares with another market maker. This is good- it means multiple market makers are entering this stock and are preparing for a volume surge- probably due to Wednesdays earnings being announced. The transaction was recorded as a "sell" transaction. If it was really a "sell" transaction the price would have plummeted. The transaction was likely an aggregate transaction of multiple share prices and the .20 price recorded was simply the last share price exchange recorded- but it could have been from a wide range of prices for a final average of .295, the closing price the day before. That is why the price did not change.
