I don't think SIRIUS can survive long term unless they do the MOST serious makeover. My reasons are:

1. They hyped the iPhone thing, but NO Howard Stern. And much of the live sports is not part of iPhone package?? That's there biggest thing by far over regular radio Stern and Sports team games.
2. Now with 3G phones anyone can hook one up to their car radio and get FREE internet radio with all the stations and music that SiriusXM plays. So why would anyone pay $12.95 to get SiriusXM that does not have it. And if you have it why pay $3 more per month when you can get internet radio (Pandora, LastFM) with even more music for free.
3. Sirius pays a lot of money to launch new sattelites into space and maintain the old ones. Very expensive way to deliver their signal. Much cheaper to use 3G airwaves.
4. Howard Stern has NOT signed on to Sirius for more years and his contract expires in 18 months. He may very well go it alone...why not...for a few thousand dollars plus the talent that he has, rent an office somewhere and create his own INTERNET radio station and then sell $3 per month subscriptions to his station or 2. That's if he stays in the business. If he does stay he may go from 4 days per week to 3 and then even less down the road. Stern is the reason SIRIUS survived 4 years ago, otherwise they'd have been bankrupt I think.
5. I get tons of drop outs on Long Island (Sirius headquarters are NYC for godsake and still not great reception nearby) and plenty of drop outs when I lived in San Diego. Too many drop outs for my sensitivity by quite a bit.
6. SiriusXM radio is NOT CD quality...I believe they broadcast music at maybe 80kbps...not bad, but not CD quality...even in the car with all that background noise its obvious the difference when I stick a CD in or iPod vs SiriusXM. SIRIUSXM is not high quality streams. Ok, but not enough to pay all that money.

The only thing they have going for them in my opinion is IF THEY GIVE UP ALL THERE SATTELITES and use the the 3G ground based repeaters exclusively for broadcasting...has to be cheaper model to keep there costs down and there charges to us...4G will be here soon..should be enough airwaves I think for radio and TV over wireless internet. And then they have to drop their price to maybe $9.99 from $12.95 per month at most....and then they have keep all their talent and keep it reasonable to be able to listen to it on multiple sources if you are the same person without paying so much extra. If I am in my car or at my office or at home, I should not have to pay 3 subscriptions...I pay one price on my and Sirius makes me log in to make sure I am not giving my account info to other people...one connection at a time ONLY. And always thru wired or wireless internet.

Conventional and Sat Radio may die out as we know it...too many other cheap ways to get stuff..but a premium service that broadcasts LIVE sports and LIVE news and LIVE music and things CD's and iPODs can't give you can survive, but its got to be real efficient and high quality and in my opinion SiriusXM is not any of those. They have to CHANGE massively. Forget the billions they wasted on all that sattelite infrastructure...there are better ways today.

CHANGE is the only thing you can count on. For SIRIUS' sake and survival, Stern is a must for another 5 years at least 3 days a week and is a MUST to be on the iPhone thru an internet connection. If not, I predict they are dead within 2-3 years. Or at least barely surviving at that point.
